Pay Range: $39/hr - $40/hour.  Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Evaluate member needs and barriers to accessing care, focusing on their priorities.
• Develop ongoing care plans and collaborate with providers to address unmet needs.
• Identify problems and provide appropriate care management interventions.
• Coordinate between members, families, and care providers to ensure person-centered care.
• Monitor member status and revise care plans as necessary to meet goals.
• Provide resource support for local services based on assessments.
• Facilitate care management and ensure timely access to needed services.
• Conduct outreach via telephonic, digital, or home visits to assess member needs.
• Maintain compliance with state, federal, and payer regulations through documentation.
• Educate members and families on disease processes and care options.
• Offer feedback to leadership on opportunities for improving care delivery.

Key Requirements and Technology Experience: 

• 2 - 4 years of related experience in care management or nursing.
• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing.
• RN - Registered Nurse - State Licensure and/or Compact State Licensure required.
• For YouthCare Illinois plan only: Bachelor’s Degree and IL RN licensure required.
• For Sunshine Health (FL) only: Minimum of two years of pediatric experience may be required.
