Key Responsibilities:
- Evaluate the needs of members, identifying barriers to accessing appropriate care and recommending solutions.
- Develop ongoing care plans and collaborate with providers to address unmet needs.
- Coordinate between members, families, and care teams to ensure person-centered care.
- Provide ongoing follow-up and monitoring of member status and progress towards care goals.
- Facilitate access to local resources for various services based on assessments.
- Perform outreach to assess member needs through telephonic, digital, or home visits.
- Maintain compliance with state, federal, and third-party payer regulations through accurate documentation.
- Educate members and families on disease processes, care options, and healthcare benefits.
- Collaborate with leadership to enhance care delivery and quality.
- 2 – 4 years of related experience in care management.
- Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing.
- RN - Registered Nurse - State Licensure and/or Compact State Licensure required.
- Experience managing high-risk pregnant women, postpartum mothers, or children.
- Knowledge of Medicaid/care-management programs.
